Now, it won't hurt to ask Vanderbilt if they would be willing to change the residency program they were going to accept you into, especially if you let them know you have had another offer in your dream career projectory. The worst they can say is no, and then you are left in no different place than you are already sitting with your present job offer! I had to do it a couple times. It's mildly worrisome, but definitely worth it. G-d has a way of allowing the right path to be the path of least resistance. That doesn't mean that we won't go through trials in this life, just that when we come to forks in the road, He shows us the path by letting things work out for His good. Keep with it friend!
